Reporting to the Director Personal Care Home the Social Worker as part of the interdisciplinary team will have accountability and responsibility for leadership, development and quality social work services to residents. Social Worker determines individual resident needs and goals through the assessment process, then develops documents and reviews individual plans of service. Social Worker assesses the residents within a holistic framework and in consideration of family and community relationships.  Social Worker accepts referrals and consultations for clients participating in the Adult Day program. Social Worker assesses the clients within a holistic framework and in consideration of family and community relationships. Social Worker will apply the concepts of Case Management for maintaining resident and client service plans. Qualifications and Competencies: ·         Degree in Social Work, Master’s preferred·         Registered with Manitoba Institute of Registered Social Workers required·         Must possess good interpersonal and communication skills·         Skill in exercising a high degree of initiative, judgment and discretion·         Experience working in multicultural setting required·         Competency with information technology such as standard business software and Social Work statistical reporting software·         Social Work experience x 5 years, Gerontology/Geriatric preferred·         Advanced education in Gerontology/Geriatrics preferred·         Encourage an environment of trust, respect and open communication·         Demonstrate critical thinking·         Utilize creativity in problem solving to maximize resident/client independence and meeting resident/client &/or family needs
