Social Worker, Acquired Brain Injury Unit

Job details:

Reporting to the Manager of the Acquired Brain Injury Unit the Social Worker is an active member of the interdisciplinary treatment team in the assessment, planning, service delivery, and evaluation of individual and group programs to meet the needs of residents in the program and their families.  The Social Worker will participate in the rehabilitation process with the Transitional Support Workers and will provide guidance and problem solving resources within the team and within the Region.  The Social Worker will deliver education, awareness and brain injury prevention information as required and will work from a recovery oriented philosophy within the Region as part of the Provincial Acquired Brain Injury Initiative for continuity of care.

 

Ÿ          M.S.W. plus one year of directly related experience; or B.S.W. plus two years of directly related experience; or other qualifications and directly related experience deemed equivalent and registration with the applicable governing body.Ÿ          Experience in Psychosocial and/or Medical Rehabilitation, Mental Health, Primary Health Care, or other related social service field.Ÿ          Well developed communication, organizational and coordination skills with competency in community development and health promotion.Ÿ          Ability to understand and integrate concepts of consumer empowerment and recovery.Ÿ          Ability to practice independently and to function effectively as a member of an interdisciplinary care team.Ÿ          Demonstrated organizational ability/critical thinking/problem solving skills.Ÿ          Well developed interpersonal skills.Ÿ          Group facilitation skills.Ÿ          Proficient in computer programs including Microsoft Word, Excel, Outlook, Publisher, and various databases.Ÿ          Ability to manage many demands that have a wide variety of complexity and size.Ÿ          Conducts professional practice as established in the Canadian Association of Social Workers Code of Ethics Ÿ          Valid driver’s license and access to vehicle. Ÿ          Good physical and mental health to meet the demands of the position.Ÿ          Subject to a Criminal Records Check and Child Abuse Registry Check.Ÿ          Must possess a dependable, consistent and reliable work record including attendance at work.
